Share your experience and help improve care

Community Mental Health Survey

Some people who use our community mental health services will soon receive the NHS Community Mental Health Survey through the post.

If you used one of our community mental health services during April or May 2026, you may be selected to take part. The survey gives people an opportunity to share their experiences of care and tell us what is working well and where we can improve.

The survey is confidential, and responses are not shared with the people who provide your care.

Your feedback helps shape our services

The NHS Community Mental Health Survey is one of the most important ways we learn from people who use our services. Feedback from last year's survey has already helped us make a number of improvements.

Patients told us they wanted:

  • more support while waiting for care
  • care plans that reflect what matters to you
  • better support in a mental health crisis
  • more support with understanding medication
  • not to have to repeat your story
  • better links with community support
  • better use of patient feedback.


In response, we have taken action in a number of areas, including:

  • expanding our Waiting Well and While You Wait support for people waiting for care
  • helping more people create personalised DIALOG+ care plans and crisis plans
  • redesigning crisis services with patients and carers, including the introduction of 111 and select the mental health option and a new crisis text service
  • launching our online Patient Engagement Portal to improve communication and appointment information
  • increasing intensive community support for people with more complex mental health needs
  • recruiting an additional pharmacist and investing in our Medicines Helpline (020 3513 6829)
  • improving continuity of care by recruiting more clinicians into permanent roles
  • expanding local support through our Be Well programme
  • developing better information about community support through Know Who To Turn To
  • launching new text message feedback to more regularly get your feedback following an appointment

We are continuing to build on this work by improving care planning, strengthening support while people wait for care, making sure we learn from patient feedback, and improving people's experience of our services.
