The Primary Care Plus service provides an enhanced level of primary care support to patients with serious mental illness operating flexibly as both a step up and step-down model. Service users include those stepped down from Southwest London and St George’s Mental Health NHS Trust (SWL) to primary care, and those who are appropriate to step up from a GP practice SMI Register.
Adults aged 18 years and over would access the service and would serve those with a diagnosis of stable schizophrenia and psychosis/Depression with Psychotic features, bipolar disorder. The service is not aimed at adults with mild to moderate anxiety and depression (clusters 1-4) as these are usually served by Talking Therapies services.

About our care
The focus of the Primary Care Plus Team is to have strong relationships with GPs to ensure that all CMHT discharges are seen within two weeks, and that an appropriate Wellness Plan (care plan) is agreed. As part of this, patient-led discussion on their areas of interest will have an emphasis on recovery via an increase in their daily activities/employment and will also provide training advice through the setting of personal goals which are reviewed every six months. Central to a recovery approach is that people should be able to build a meaningful life for themselves, and that they should be supported to take control of and manage their own condition, so engagement with carers, family and friends, particularly at the initial phase of discharge from the CMHT is essential.
We also offer help in accessing community services such as Active Wellbeing, Recovery College, Hestia Recovery cafes, Carers Centre , EMHIP -NTA Hub, Family Action, Choice Support (Employment related ).
Who is this service for?
The service is for adults ageded between 18 and 75 years old with severe and enduring mental health problems (SMI-Serious Mental Illness), who live in and are registered with GPs in Wandsworth and currently not under any Secondary Mental Health Services.
Referrals and access
Your GP should always be your first point of contact for any mental health related issues.
Your GP can refer you to our service for ongoing mental health support and to ensure you stay well and safe in the community.
Secondary Mental Health Services such as the CMHT’S , Home Treatment Teams, Early Intervention Service , Wandsworth Single Point of Access and the Psychiatric Liaison Teams etc can refer you to our service in primary care.
We also contact service -users who have already been discharged and in the community to introduce them to the service and invite them for informal chat about their mental/physical wellbeing and the available support to enhance their recovery.
