About our care
The service provides a wide range of support to adults with learning disabilities who have challenging behaviour and other mental health issues.
The team is made up of consultant psychiatrists, clinical psychologists, nurses and behaviour analysts. They focus on working with patients in community settings and supporting transitions back into the community when patients leave hospital.
A range of psychological treatments and interventions are offered including a framework called Positive Behaviour Support (BPS).
Positive Behaviour Support
Positive Behaviour Support (PBS) is a person centred framework for providing long-term support to people with a learning disability, including those with mental health conditions,
who have, or may be at risk of developing, behaviours that challenge.
It is a blend of person-centred values and behavioural science and uses evidence to inform decision-making. Behaviour that challenges usually happens for a reason and may be the person's only way of communicating an unmet need. PBS helps mental health professionals to understand the reason for the behaviour so they can better meet people's needs and reduce the likelihood that the behaviour will happen.
PBS has an overall aim to improve quality of life, and to provide the right support at the right time, so that people can thrive, be happy and fulfil their potential. PBS means treating people equally and working in partnership with people with learning disabilities and their families.
Who is this service for?
This service is for adults with a learning disability who are under the care of a GP local to Wandsworth.
Referrals and access
Local GPs should be the first point of contact for adults with learning disabilities who require mental health support.
GPs can provide referral to the Wandsworth Learning Disabilities Healthcare Team via the NHS e-Referral Service (e-RS).
Other healthcare partners and agencies can refer to the team by emailing WandsworthMHPHLD@swlstg.nhs.uk.
