About our care
The service plays a crucial role in the early identification and management of memory-related conditions, promoting timely intervention, and providing necessary support for individuals and their families.
If an individual is diagnosed with dementia following an assessment, a personalised care plan will be developed to address their specific needs. Referrals will be made to various support services and organisations, and education and guidance on coping strategies, lifestyle adjustments and community resources may be provided.
Regular follow-up appointments are scheduled to monitor the individual's cognitive function, adjust their care plan as needed, and provide ongoing support.
Those assessed to have a mild cognitive impairment will be offered a yearly review.
Who is this service for?
Adults aged 50+ (or below in exceptional circumstances) who live in Sutton.
Referrals and access
The service accepts referrals from GPs (via the e-Referral Service) as well as geriatricians and neurologists.
