About our care
The Service includes different professionals: consultant psychiatrists specialising in supporting older people, community psychiatric nurses, psychologists, occupational therapists, and recovery support workers. Patients are seen in outpatient clinics or in their own home.
Depending on needs and circumstances, patients are supported by one of the following teams, with easy transition between teams if required.
Merton and Sutton Challenging Behaviour Service:
The challenging behaviour services provide assessment, treatment and support to people who typically have a diagnosis of dementia.
Merton Older People's Memory Assessment Service:
Memory Assessment Services provide diagnosis and treatment of dementia in people over 75 years of age (or below in exceptional circumstances). Our Memory Assessment Services also help to provide signposting to other services for post diagnosis support once a diagnosis has been made in one of our clinics. Referral to our Memory Assessment Services is usually via a service user’s GP as well as Secondary Care providers such as geriatricians and neurologists.
Merton Older People's Community Mental Health Team:
Our CMHT’s/ RST’s are for service users who are generally 75 years and over with moderate to severe mental health conditions such as mood disorders or psychotic illness, using a recovery model approach. We work closely with their carers, relatives, and social services to help facilitate joint and inclusive care planning. Referral to our CMHT’s/RST’s is usually via a service users’ GP as well as other Secondary Care providers such as geriatricians and neurologists.
Who is this service for?
Adults in Merton who are over the age of 75 or of any age with a diangosis of dementia, including residents in care/nursing homes.
