Merton Primary Care Liaison Service (PCLS): Merton Single Point of Access and Merton Primary Care Liaison

  • Merton Primary Care Liaison Service (PCLS) is the central point of access for all adults referred to community mental health services in the borough. The service provides initial assessment, screening, triage and signposting to a range of services, as well as time limited recovery-focussed and wellbeing interventions.  

    The service formed in April 2024  as a result of Merton Assessment Team (MAT) and the Primary Care Recovery Service (PCRS) merging.

About our care

Merton Primary Care Liaison Service (PCLS) has two distinct functions: 
 
1. Single Point of Access for clinical screening, triaging referrals and conducting psychiatric assessments to determine appropriate treatment and interventions based on service user and carer needs. 
2. Primary Care Liaison function to work closer with Primary Care (including GPs) to provide a range of brief time-limited interventions. This includes medication initiation, observation of response to treatment, physical and social support and community engagement and signposting. The service also supports service users to transition back into Primary Care following an intensive period of support within specialist mental health teams.  

 

Who is this service for?

The service provides referral and signposting support for adults aged 18-74 in the London borough of Merton.

Referrals and access

Self-referrals  

If you would like to refer yourself to the service, p lease use the self-referral form.  A family member, carer of friend can also make a referral on behalf of somebody else.

GP Referrals  

If you are a professional working in a GP practice in Merton, please submit referrals using the electronic referrals service ( e-RS ). We cannot accept referrals from GPs in the borough if they sent via other methods.   

Out of area of area GP referrals should be sent to mertonspa-pclsduty@swlstg.nhs.uk 

Professional referrals (external organisations )  

If you are a professional from any other external organisation (for example, hospitals, third sector organisations, out of area GP practice staff, police etc.) , please send referrals to  mertonspa-pclsduty@swlstg.nhs.uk using the referral form provided by the service. If you do not have the referral form, please request this by contacting the email address above.   

Professional referrals (internal teams at South West London and St. George's)  

All internal referrals should be sent directly to the treatment team (not Merton PCLS ) using internal referral processes on the clinical system. Any internal referrals that are made using other methods will not be accepted.

Merton Assessment Team is a key pathway to community mental health services.

The service does not provide treatment for mental health issues, but rather offers initial assessement, screening, triage and signposting to a range of services which do.

It is made up of a team of professionals who combine their skills and includes team managers, consultant psychiatrists, referral triage assessors, occupational therapists, nurses and social workers.
