About our care
Referrals made to the service are screened by the multidisciplinary team. Depending on the referral information received, the service may need to gather more information, for instance requesting reports relating to the diagnosis of a learning disability or more detail on health conditions. The outcome of this screening will be feedback to the referrer and the individual.
If eligible for the service, an assessment will be offered to determine the best way to support adults with learning disabilities and their families and carers.
The service is made up of a group of different mental health professionals who combine their skills to provide a range of recovery support to enable people with intellectual disabilities experiencing mental ill health to live well and stay well in the community.
This includes psychiatrists, psychologists, social workers, occupational therapists and health support workers. They provide support in residential settings and at the Trust's local outpatient centres, the Jubilee Health Centre in Sutton and the Wilson Hospital in Merton.
The team work support health and social needs and provides a range of therapy and treatment including psychological, behavioural, and family therapy, and medication options.
Key interventions include:
• Psychological therapies including counselling or cognitive behavioural therapy
• Psychiatric interventions including medication therapy
• Care coordination for service users with complex mental health needs
• Consultation to mainstream services this includes guidance around reasonable adjustments
