About our care

Kingston Older People's Community Mental Health Team, Memory Service and Care Home Liaison Team is made up of a group of different mental health professionals who combine their skills to offer support to older adults in the community. The team provides support for people experiencing severe mental health issues as well as conditions such as Alzheimer's, dementia and memory problems.

Support is available from a range of different professionals depending on individual needs. This includes consultant doctors specialising in old age psychiatry, social workers, community psychiatric nurses, psychologists, occupational therapists, and recovery support workers.

These services are underpinned by a psychological approach to managing challenging behaviour. Some teams provide an outreach service for service users in their own home, supporting them and their carers, while other teams work exclusively with service users and carers in formal care settings such as nursing homes and residential homes.

Who is this service for?

The service typically provides support for people aged 75 and over with a severe mental health issue, Alzheimer's, dementia or a memory problem. It also supports people who may already have a diagnosis of dementia and have additional mental health issues.

Support is offered to younger people where frailty is a level where their needs are best met by this service. Similarly, the service is available to people younger than 75 where memory issues are the primary concern. 

Referrals and access

Local GPs in Kingston make a referral via Referral Service (e-RS).
GPs may also refer patients to specialists such as geriatricians and neurologists who can make a referral to this service.

Patient information

People referred to the service will work with the team to develop a plan for their care and support at the outset.

Alongside this, a named healthcare professional will help organise, manage and monitor this plan and will act as a main point of contact.

For those with more complex needs a dedicated care coordinator will provide this support
