About our care
The Forensic Outreach Service provides specialist mental health support and interventions for individuals who are transitioning from a forensic inpatient service back into the community. It includes medication management, psychology groups and 1:1 sessions, and risk assessment and mangement.
The primary goal of the service is to support recovery and promote social inclusion.
Who is this service for?
Individuals stepping down from secure inpatient wards who are entitled to aftercare in south west London.
Referrals and access
Individuals are typically referred by the inpatient service providing their care and treatment.
