About our care
The Forensic Outreach Service (FOS) is a specialist, multi-disciplinary service operating within Forensic Services to facilitate and manage the safe and efficient transition of high-risk adults with mental disorders from secure forensic settings (medium and low secure care) into the community, and back into their appropriate Community Mental Health Team or GP.
FOS also provides expert advice, a responsive consultation and a liaison service to other mental health services and relevant stakeholders (probation, police, security services, courts, Prevent, etc.) in the management of service users with mental disorders who have offended or are at risk of offending/reoffending.
Who is this service for?
Service users accessing FOS typically but not exclusively include those who have a serious and ongoing risk of violence, sexually inappropriate behaviour, or other harm to others that is linked to their mental disorder and are:
- subject to MoJ restrictions and are Conditionally Discharged
- subject to license conditions with a mental health disorder as discharged by the Parole Board
- subject to Community Treatment Orders (CTOs)
Service users accessing FOS typically but not exclusively include those who suffer from:
- severe and enduring mental illness
- personality disorder with complex needs or who fulfil the definition of a ‘dangerous and severe personality disorder’ who are amenable to treatment
Referrals and access
FOS will accept referrals for individuals who are the responsibility of the boroughs within South West London and are stepping down from a secure forensic inpatient ward.
Referrals need to be sent via the SLP at slm-tr.slpforensicreferrals@nhs.net
