Our Service provides sign-posting, health monitoring, support and education for people with a diagnosis of Dementia, and their families or carers with a GP in Wandsworth, and who are not already open to any other secondary Mental Health Service.

About our care
This service is for people diagnosed with dementia and/or their families and carers who have been identified as needing additional help and support.
The team is made up of nurses who have specialist knowledge and expertise in dementia and associated issues, as well as knowledge of relevant local support services and organisations.
The team provides health monitoring, education, and signposting, enabling individuals with dementia and their families/carers to access services that help maintain their physical and mental wellbeing.
A comprehensive initial assessment and follow-up appointments will take place at home by an allocated nurse. This may include advice and support around nutrition and hydration, elimination, mobility, sleep, skin integrity and footcare, mood, mental state and memory, medication, and any upcoming health appointments or travel.
Who is this service for?
This service is for individuals with a diagosis of dementia who live in Wandsworth. They should not be receiving care from any other secondary mental health service, and they should not display behavioural and psychological symptoms of dementia (BPSD).
Referrals and access
GPs and other healthcare professionals can make a referral via the e-Referral Service (e-RS).
