Richmond Older Adults Services

  • Richmond Older Adults Service is a community-based service for adults aged 75+ who are experiencing mental health difficulties. that cannot be supported in primary care. We provide assessment for a potential dementia diagnosis and post-diagnostic support for people of any age.

    The Service comprises of three teams who work together to provide appropriate care and support according to patient or carer needs. There is also strong collaboration with colleagues in other healthcare services, social care and the voluntary sector.

About our care

The Service includes different professionals: consultant psychiatrists specialising in supporting older people, community psychiatric nurses, psychologists, occupational therapists, and recovery support workers. Patients are seen in outpatient clinics or in their own home.

Depending on needs and circumstances, patients are supported by one of the following teams, with easy transition between teams if required.

Richmond Care Home Liaison Service:

Our challenging behaviour services are generally for service users who already have a diagnosis of dementia. However, some of our services also help service users with other mental health needs too. These services are usually underpinned with a psychological approach to managing challenging behaviour. Some of our teams provide an outreach service for service users in their own home supporting them and their carers. While other teams work exclusively with service users and carers in formal care settings such as nursing and residential homes. Referrals sources include primary care, secondary care and, in some boroughs, local providers.

Richmond Memory Assessment Service:

Memory Assessment Services provide diagnosis and treatment of dementia in people over 55 years of age (or below in exceptional circumstances). Our Memory Assessment Services also help to provide signposting to other services for post diagnosis support once a diagnosis has been made in one of our clinics. Referral to our Memory Assessment Services is usually via a service user’s GP as well as Secondary Care providers such as geriatricians and neurologists. 

Richmond Older People's Services - Recovery Support Team: 

This service is for people who are generally 75 years and have moderate to severe mental health conditions such as mood disorders or psychotic illness, using a recovery model approach.

Referrals and access

The service accepts referrals from GPs via the e-Referral Service (e-RS).
