The Home Treatment Team are a community based mental health service. We provide crisis and home treatment support to those experiencing a Mental Health Crisis. The team provides assessment and treatment in the community as an alternative to coming into hospital, so people can receive the care they need in their own home

About our care
The team provides care and support in the community as an alternative to a hospital admission. The service supports people in crisis to stay in their own home with increased support. Care and treatment is short term (up to six weeks). The team will work with patients to agree a care plan which will identify what recovery goals are and how we best offer support.
The care plan varies for each individual and the multi-disciplinary team will work closely together to provide seamless care and agree on longer term care provisions either from a GP or other relevant services.
The team see patients initially on a daily basis. Depending on your progress the team will adjust the frequency of the visits whilst gradually working towards a discharge plan. The team will also offer ongoing assessments to help people through the crisis and stay out of hospital. They will work with you right through to the point of discharge, and will work with you to develop a holistic plan of care and support (collaborative crisis plan).
They can also refer you to appropriate services for follow up care and support after the crisis. They treat the following conditions - Anti social and Personality disorders, Anxiety disorders in Adults, Bipolar disorders, Depression, psychotic episodes and Schizophrenia
Team members come from different professional groups including nurses, doctors, occupational therapists, recovery support workers, peer support workers with lived experience and social workers. We would work closely together to provide seamless care.
Who is this service for?
This service is for Merton patients who are over 18. The patient will have a severe mental illness and will require support at home due to severity of symptoms and clinical risk factors
Referrals and access
Referrals can be received from Recovery Support Teams, Psychiatric Liaison Services, Single Point of Access and Inpatient wards. Clients can self refer if they are within 14 days of being discharged from HTT.
